I know its not from a concert, but I have Shane Embury's guitar he used to write all of Brujeria's old songs on and recorded their new album that is yet to come out on it. Its a 7 string ESP Viper

I've been to well over 200 shows at this point. And usually am in the pit or on the barrier by the end of the show. Over time i have somewhat learned tendencies and what to watch for. Usually if you make eye contact with a guitarist at the end of a song you can get them to throw a pick to you. That's what I did when I caught twiggy's pick. He made a perfect throw from about 15 feet away too. And I've turned catching picks out of the air into an artform. Some have been lucky like the slipknot one. Bounced off a couple people and landed at my feet. Some have been how the fuck did that happen. Such as the underoath drumstick. I was talking to the person next to me and motioned with my hands something to the person hands out palms towards the sky and the stick landed perfectly in my hand.

I have never got a pick, drumstick, or setlist. I have come so fucking close so many times to catching picks or drumsticks it just hasn't worked out ( During Unearth's set at Mayhem I thought I was going to catch Ken Susi's pick then this kid came out of nowhere and got it. When I saw In Flames headline two years ago a pick was coming at me, I lost it in the lights, then it landed it behinded and some chick grabbed it right when I found it.)

Honestly, towards the end of shows/sets, I ALWAYS forget about setlists. Picks and other stuff I just haven't been as lucky. I almost got a setlist from the Amon Amarth show (a friend of mine got two of them) but I left the venue before he could get to me.
